Mrs X, 40yrs old petite lady married since 12 yrs with regular but scanty periods since menarche came to us for fertility treatment.

Pelvic scan showed less than normal size uterus with AFC of 2/1. She was planned for IVF with Donor egg. On Hysteroscopy, our concern was confirmed on finding a small tubular Uterus with a short conical cervix and pinhole external os.

We had a real challenge in not only making her conceive but to continue her pregnancy till near term when we can give her a viable baby. For this, we put HRT medications for 3 months. After that, Single Embryo transfer was done considering the size of the uterus and she became pregnant.

Ahead of us was a mammoth task of making her carry her pregnancy till term. She was closely monitored and at 13wks Cervical Stitch was applied as a safety measure to avoid miscarriage. At 31wks , Emergency LSCS was done because her BP shot up and we delivered a live preterm female baby weighing 1.7 kg. Mother and Baby are doing well.
